Your photos and application form will be reviewed through the Seattle office.  If approved and accepted you will be contacted by mail within 1 week.   If you do not have a working printer - just write down the information and send with photo(s).

READ VERY IMPORTANT INSTRUCTIONS BELOW
  • You may send color copies, professional photos, or recent snapshots.
  • Photos should fit in a 6"x9" or smaller envelope.  Do not send 8"x10" or boxes.
  • Send $5.00 for processing of your "off line" application. US Cash or Money Order
  • DO NOT SEND: Overnight packages - it will delay your application for up to a week.
  • DO NOT SEND: Videos, CD's, or Disks.
  • DO NOT SEND: ANYTHING LARGER THAN 6" x 9" (It will not NOT be opened or returned)
